A MEFV result must be interpreted at the level of the exact variant and the person’s clinical findings. Counting variants or searching for a familiar gene name is insufficient. This guide separates the molecular evidence, the inflammatory phenotype and the care decisions that follow professional assessment.

What is pyrin?
MEFV encodes pyrin, an innate-immunity protein involved in inflammasome assembly and regulation of inflammation. The gene’s biology explains why it is investigated in periodic inflammatory illness, but a plausible pathway does not prove that a particular variant explains a person’s symptoms. [1]
FMF often involves recurrent fever and painful inflammation, with episodes beginning in childhood or adolescence, although later presentation occurs. Ancestry can inform the assessment but should not be used as a stand-alone diagnostic rule. [2]
Read a MEFV report in six steps
- Referral question: record why the test was ordered and the main observed findings.
- Exact identity: copy transcript version, DNA notation and protein notation.
- Classification: retain the laboratory’s category and date for each variant.
- Gene copies: note zygosity and whether the arrangement of two findings is known.
- Scope: identify whether testing covered selected variants, the gene or a broader panel.
- Conclusion: separate the laboratory finding from the clinician’s diagnosis and follow-up plan.
A phrase such as “one MEFV mutation” leaves several questions unanswered. It does not identify the variant, its classification, the test’s scope or the inflammatory findings. Begin a consultation with the actual report and a symptom timeline instead of an online interpretation of that phrase.
What does one detected variant mean?
Most people heterozygous for an FMF-associated pathogenic variant remain asymptomatic. Some have inflammatory manifestations, and certain variants can be associated with distinct dominant pyrin-related disorders. Interpretation therefore depends on the particular finding, family history and clinical presentation. [3]
Do not automatically dismiss symptoms because only one variant was found, and do not label every heterozygote as having FMF. Ask what explanation the specialist considers most likely and what evidence would distinguish alternatives. These are questions for an integrated assessment, not a decision tree that a website can complete from a gene symbol.
What about two findings or uncertain variants?
Biallelic pathogenic MEFV findings with suggestive clinical features support the molecular diagnosis described in GeneReviews. Uncertain variants, including one uncertain finding paired with a pathogenic finding, do not independently establish or exclude FMF. [3]
For two different variants, ask whether their phase is known: are they on opposite gene copies or together on one copy? Record “unknown” if the report does not settle that question. Do not replace the laboratory’s conclusion with a simple count of how many rows appear in the results table.
The classification framework guide explains why different evidence types carry different weight. The ClinVar disagreement guide helps compare assertions for the same variant and condition without treating the most alarming label as automatically correct.
Build an evidence table that can be checked
Use separate columns for the exact variant, population observations, reported clinical features, family evidence, functional experiments and interpretation date. Identify overlapping cohorts so that one group of participants is not counted repeatedly through several publications. Mark whether a source presents original observations or summarizes earlier work.
For an inflammasome experiment, record the cell system, stimulus, comparator and measured endpoint. An increase in an inflammatory readout may be mechanistically interesting without quantifying clinical severity. Read the functional evidence guide before translating an experiment into a diagnostic claim.
Does the genotype determine treatment by itself?
EULAR/PReS guidance centers management on clinical disease, inflammatory control, treatment adherence and tolerance. A genetic result should not be used alone to start, stop or adjust colchicine or biologic therapy. Discuss the complete clinical record with the treating specialist. [4]
A useful care handover has three short sections: what the molecular report establishes, what the clinical assessment establishes and what remains uncertain. Include the current plan and review date. This makes it harder for an uncertain variant to become a definite diagnosis through repeated copying between services.
How should a research cohort be described?
Specify the diagnostic criteria, variants, phase where relevant, ancestry description, clinical manifestations and treatment context. Keep asymptomatic heterozygotes separate from clinically assessed inflammatory disease. When analyzing severity, state exactly what was measured and over what period rather than use a single undefined “severe” category.
For a student project, compare how two studies define an attack and handle missing diary entries. Then consider whether their outcomes can be combined meaningfully. This exercise often reveals a methodological difference before a biological explanation is needed. The gene–disease validity guide provides additional context for evaluating a claimed association.
